(SHEBOYGAN, WI) – Your thoughts matter! You know your community better than anyone. Local health partners, including Sheboygan County Division of Public Health, Aurora Health Care, HSHS St. Nicholas Hospital, Froedtert Health, Lakeshore Community Health Care, and the United Way of Sheboygan County are urging residents to participate in a community health survey. The Community Health Needs Assessment (CHNA) survey will provide valuable data for continued evaluation of community needs and help guide government, not-for-profit, and volunteer organizations as they fashion services to meet the most pressing health needs of the community. In order for as many Sheboygan County residents to have their voices heard, an online survey available in English, Spanish and Hmong will be available to anyone who lives and/or works in Sheboygan County. This anonymous survey will take about 10 minutes to complete and is open to anyone 18 years and older. Plus, you could win a $50 Walmart gift card just for completing the survey! No one will ask for identifying information, such as social security number, banking account numbers, or other personal identifiers during the survey. Questions will focus on issues such as access to primary care, health care coverage, substance use, smoking, nutrition, physical activity, cancer prevention, injury prevention, immunizations, children’s health, mental health, and chronic disease. The new data will supplement 2023 community health assessment data and will allow Sheboygan County Public Health and community partners to analyze trends over time. Results will be released to the public later this year. The community health survey is created to: ● Gather local data to document health status related to health priorities established by the state health plan. ● Identify local health behaviors or risk factors that present opportunities for health promotion and health care cost reduction ● Track the success of community health improvement activities 1 ● Allow local health partners to document population health indicators to help guide health improvement efforts. The survey is available on the Healthy Sheboygan County website. This survey is open from now through the end of April.
